1. Membership cards must be produced when requested by a Sydney Uni Sport & Fitness staff member. No card, no entry.
2. Unauthorised entry will not be tolerated. Unauthorised entry includes but is not limited to: sharing passes, allowing a non-member unpaid entry, entry by emergency exits, use of invalid/expired membership cards and not wearing a valid wrist band. Unauthorised entry represents theft, trespass and breach of OH&S regulations.
3. Full-length towels must be used by members on all gym equipment at all times. Members without towels will be asked to hire one from reception. No towel - no training.
4. Closed-in sports shoes must be worn at all times for all activities in all Sydney Uni Sport & Fitness Facilities (except swimming). Boots or black-soled shoes are not permitted in the sports hall and squash courts.
5. Appropriate attire to be worn at all times - shirts must stay on (except when swimming).
6. Offensive language, abuse of staff or other facility members, will not be tolerated and offenders will be asked to leave the premises.
7. Facility members are to follow the instructions of Sydney Uni Sport & Fitness staff at all times.
8. Abuse of, or damage to, any equipment will not be tolerated and offenders may be asked to leave the premises.
9. All weight plates and equipment must be replaced to storage racks/ areas upon completion of use.
10. No food is to be eaten in any of the Sydney Uni Sport & Fitness Facilities except in the designated bistro areas.
11. To be eligible for credit, patrons must give twenty-four hours notice for transfer of court bookings.
12. No alcohol consumption is permitted unless special permission has been granted by Sydney Uni Sport & Fitness management. Any persons found to be under the influence of alcohol or drugs will be asked to leave the premises.
13. Any member caught stealing will have their membership terminated, will not be allowed back on the premises, and will be referred to the appropriate authorities.
14. Sydney Uni Sport & Fitness Facilities members must be over the age of 17 (with the exception of Sydney University undergraduates and school groups) unless special permission has been granted.
15. Children may swim, play tennis, squash and casual basketball but must be supervised by a responsible adult of 18 years or over. Guest fees apply.

All Sydney Uni Sport & Fitness Facilities patrons must complete a registration form or they will not be permitted to use the facility. Patrons must be current annual members of Sydney Uni Sport & Fitness to use the facilities.
A variety of pass purchase options are available to all members and non-members, including a monthly billing option. Please see the special terms and conditions of the monthly billing option as conditions do apply. Prices are GST inclusive.
When a membership expires, so do the services originally included. Please choose carefully, as there are no refunds except in special circumstances (see below). 10-visit passes expire one year from the date of purchase (Gold Lion Pass holders exempt).
All Sydney Uni Sport & Fitness Facilities abide by the Fitness Industry Code Of Practice in all matters, including its Terms and Conditions. A copy of the Code is available at reception. Alternatively, you can contact Fitness NSW on (02) 9460 6200 or www.fitnessnsw.com.au.
Membership cards must be shown on every visit. Lost cards must be replaced. Replacement card fee is $7.50 (incl. GST).
Memberships are not transferable i.e. they cannot be used by anyone other than the member whose name appears on the card. Abuse of membership privileges will result in immediate termination of membership.
There is a seven (7) day cooling off period for all prepaid term memberships of three months or longer. Any refund request must be made in writing to the Centre Manager, during the cooling off period, and will be subject to a deduction of an administration fee (see below) and fees for the services already received. Except for permanent physical incapacity, which must be validated by a certificate from a registered medical practitioner, membership is not refundable for any other reason or circumstances beyond Sydney Uni Sport & Fitness' control.
A decision to offer a refund will be at the sole discretion of Sydney Uni Sport & Fitness management. Where a refund is offered, we agree that it will be paid by cheque within fourteen (14) days of the claim being received. A $25 administration fee will be charged, along with a deduction for the value of any services received (including visits, fitness assessments and/ or personal programs) (all GST inclusive).
The seven (7) day cooling off period does NOT apply to: 10-visit passes; Any subsequent purchase of any prepaid term memberships including renewals.
A time stop form must be completed by the member prior to the period of absence otherwise the time stop will not be valid. Gold pass holders receive free time stop. For other pass-holders a time stop fee of $10 is due at time of request. Time stop privileges are available for a minimum of ten (10) days and a maximum of 42 days only for a 12 month pass, 21 days only for a 6 month pass and 10 days only for a 3 month pass.
A time stop may be considered on other term memberships of three (3) months or longer for reasons of temporary physical incapacity. In the case of temporary physical incapacity, the member must complete a time stop form within 8 weeks of its occurrence with a certificate validated by a registered medical practitioner. Time stops will only be granted for the time specified in the certificate. A temporary physical incapacity can be defined as "an injury or ailment which prohibits the individual from participating in any kind of physical activity offered by the facility, for a period of up to 12 months".
A decision to offer a time stop will be at the sole discretion of Sydney Uni Sport & Fitness management. Annual memberships cannot be put on hold.
By signing this registration form, you have agreed to abide by the terms and conditions set out by Sydney Uni Sport & Fitness management in relation to the use of its facilities, to conduct yourself appropriately and to follow the reasonable directions of staff whilst you are on or about the premises. You acknowledge that if you fail to comply with any such rules or directions and otherwise engage in behaviour which is offensive or disruptive to Sydney Uni Sport & Fitness' Facility members or other patrons, you may be asked to leave the premises and/or have your membership cancelled. You are aware that in such case, no refund of membership will be paid.
Breaches of the above Terms and Conditions will result in suspension or cancellation of usage privileges at the sole discretion of Sydney Uni Sport & Fitness management.
All personal information collected by USYD, Sydney Uni Sport &Fitness, all affiliates, clubs and agencies is collected in accordance with the National Privacy Principles contained in the Commonwealth Privacy Act 1988 as amended 2000. This information will not be disclosed to any unauthorised third parties without your consent. If you do not complete all information requested on this form, we may not be able to process your membership. Sydney Uni Sport & Fitness reserves the right to use the information collected for promotional purposes unless otherwise notified.
